Re-Certification Application for Cardiovascular Nursing and ECG Board Certification Status

You may print and mail your completed re-certification application or if time is important, copy items 1-17 below and paste in the body of an email, fill in the information in your email, and send to our Exam Director Jonni Cooper at:   jcooper17@tampabay.rr.com. 

At the same time, copy and mail each of the CE/CME certificates you are claiming for re-certification (50 CME/CEs are required for Cardiovascular Nursing Re-certification, and 30 CME/CEs are required for all ECG Re-certifications). 

Your re-certification letter will be emailed as soon as we receive proof of your CME/CE credits, your payment, and the answers to items 1-17 in this application.

You may pay for your re-certification online, or you mail a personal check or money order with your completed application.   

Recertification applications will be processed on Tuesdays and Fridays.  The exam offices are closed weekends, and major holidays. 

For emailed re-certification applications, this process should not take more than 3 business days to complete.

For mailed re-certification applications, this process should not take more than 5 business days to complete.
